Help understand engine codes

Just picked up the car back friday night and noticed the car just doesn't seem to be running properly. When you step on the gas the car just can't seem to get out of the way of itself. The car seems to idle properly however it doesn't seem to start the same way it used to. In the past on a cold start it would first rev high, the idle would drop low and almost seem to stall (on real cold days it would) then it would bring the RPM's up again and idle properly (I thinking the IAB might need to be cleaned to fix this issue). Now it just brings the RPM's around 800. I pulled the codes and got the following

KOEO:
11 - OK

Memory
41:
From Phoenix SHO project: System was lean for 15 seconds or more (no HO2S switching) - Fuel control
From SHOTimes : HEGO sensor circuit indicates system lean (rear).

KOER
26:
From Phoenix SHO project: Mass Air Flow (MAF) or Vane Air Flow (VAF) out of range - MAF VAF or Transmission Oil Temperature (TOT) sensor out of range - Transmissions
From SHOTimes: NOT LISTED
41:
From Phoenix SHO project: System lean - Fuel control
From SHOTimes: HEGO sensor circuit indicates system lean (rear).
13:
From Phoenix SHO project: Idle Speed Control motor or Air Bypass not controlling idle properly (generally idle too high).
From SHOTimes: Cannot control RPM during self-test low RPM check. Suspect idle bypass valve problems.
25:
From Phoenix SHO project: Knock sensor not tested (ignore if not pinging) - KS
From SHOTimes: Knock sensor (KS) not sensed during dynamic response test.

The 26 could be from not bringing the car up to normal operating temp. I did drive the car for a little before doing the test but the car did sit for a little once I got back home before I did the testing. I also couldn't get the cylinder balance test to run. I'm going to go out there and try it again.

Additional information

Ran the car for a while and re-ran the KEOR tests. Here are the resutls. All three passes were run in succession.

The cylinder results really worry me. I guess it is indicating a problem with all the cylinders on the left (rear) of the car? :scared: The fun just doesn't end. Any help would be greatly appreciated as i don't know what to make of the results. Thanks.

Dissconnect battery, turn on head lights and push and hold brake pedal for 10 seconds. Reconnect battery and drive it for several miles to allow PCM to fully reset. Now run KOEO and KOER test.

That was one of the suggestions I found while searching the forum and I gave it a try. Unfortunately the codes (and the performance) I got were basically the same as before

Dissconnect battery, turn on head lights and push and hold brake pedal for 10 seconds. Reconnect battery and drive it for several miles to allow PCM to fully reset. Now run KOEO and KOER test.

Agree you need to perform the idle reset procedure. Follow procedure above, except also turn A/C to Max Cool before starting engine, and turn headlights on immediately after it starts. This is to apply maximum accessory load while the PCM recomputes your idle schedule.

Hey guys,

Yep, ran through the reset procedure twice with the same result. Any ideas on what would cause this lean condition across all cylinders on the left bank? I've seen people in other posts say the injectors or a vaccuum leak. But it seems odd to me that all the injectors would go at once or if it was a vaccuum leak I would think the lean condition would exist across all the cylinders.

Just some additional info, the car has all new plugs, wires and coil pack.
