Car barely runs!

I'm still having problems with my 93 ATX. I have problems starting it (I need to use starting fluid) and when it's running it won't idle and sounds like it's running on only a couple of cylinders. I found that if I can get it rolling I let it rev up around 4000-5000 RPM and something frees up and the car goes back to normal. If I let it idle for a minute or so it starts to run ruff again and if I turn it off I have trouble restarting it. Any ideas what the problem could be? The first couple of times this happened the problem went away, but now it won't start without the starting fluid. I checked the codes a couple of times and got different readings each time. I got the codes for the MAF multiple times so I tried replacing it but that wasn't the problem. I cleared the codes, ran the car, and rechecked them a couple of times and I seemed to get different codes each time. Here is what I got.
KOEO tests:
1st try (engine not at operating temp): 116, 636, - 158, 172, 176, 533, 542, 556
2nd try: 111 - 158
3rd try: 111 - 122, 157, 158, 542,
KOER test:
Note - I had to keep my foot on the gas to keep the car running during the test because it runs so poorly.
Only try: 116, 136, 129, 167, 225

Even though I didn't get any codes for cam or crank position sensors are these posibilities? It almost sounds like it's out of time.

Any help will be greatly appreciated.

I had a similar problem. Problems were slightly milder than yours but they were the same problems. All I had to do was clean out my throttle plate and intake. Ran great after that. It was one of those weird problems that seems like it's going to cost a ton of money to fix but ends up being quite easy.. Hope it's the same for you. Good luck

I cleaned the intake and throttle body about 5000 miles ago when I did the 60K service, but it can't hurt to check/clean it again. Thanks.

Are there any other ideas of what the problem could be, if this isn't the problem.
