94 ATX Tranny woes

So My transmission has been acting up huge on me, If I shift it manually from first to second it is fine at low throttle. When I put it into D it will shift from first to second easily at low throttle then if you have it at WOT it shifts from first to second lazily. It will down shift from second to first but will not come out of second gear no matter what you can hold it at redline and it will not shift. Now I have been haviong problems with it before this. O/D light flashes and it wouldnt lock up into OD.

Ran tests and this is what I got

327- EGR circuit was low

646- First gear failure
647- Second gear failure
648- third gear failure
659-?
645-?
