How does it run off idle?
Have you tried unplugging the maf?
It "should" run with the maf unplugged. Does it have a stock maf? Any kind of chip or LPM in the PCM?
Pull the chip out and see what happens, then unplug the maf and try again.

There's a cover for the interface at the PCM/ECU, comes from the factory. But probably lost a while ago. Unhook the battery, pull the plug from the "firewall" side of the LPM, hook the battery back up, and perform the idle reset procedure.
Yes, unplug it at the gray end of the ribbon cable.
I would disconnect the battery for the chip
