Your ECT is most likely bad. If the computer can turn the fans on during the KOEO self test, but not under normal driving, it doesn't know its getting hot. The ECT is probably bad. Is your fuel economy poor as well? Does the fan turn on if you turn the A/C on?

The fans aren't kicking on with the a/c on. But the fuel economy might be a little off. the last time I checked it was like 20, but that was with the fans working correctly.

Yup, it's the ECT sensor. Replace it. Make sure you get one with a BRASS probe, not a plastic one. The plastic ones are junk and will last a few thousand miles at best.

Check your relays for the fan as well... there are more than one. Also...when the A/C is on, is it actually working? Is the compressor kicking in?

The ECT is the Engine Coolant Temperature sensor, located under the throttle body. It sends info to the computer to tell the car to go into open or closed fuel loops and tell the fan to kick on when the car gets to a certain temp. Just do a search and you will find a wealth of info.

I'm with Chris (AutoSHO) on this one. If the fan kicks on during the KOEO test but won't activate while you're driving, then the relays are most likely fine and not to blame. I'd change your ECT sensor first and then see what happens afterwards.

I agree, it is most likely the ECT. I had the same problem last year, my fan would never come on, even when I turned on the AC. (never tried the KOEO test.) But I replaced my ECT and still no fan.
Then I replaced the ICRM (it contains the relay for the fan.) After that it worked perfectly. But since your fan turns on during the test, the relay is probably good.

In your earlier posts...

116 (O,R) Engine Coolant (ECT) sensor out of range - ECT

Did you replace the correct sending unit? The two-wire and not the single wire for the dash gauge? Can you provide us with a part # from the part invoice?
