Sorry to hear it got torn down again with no obvious problem.
How was the TOB? Any abnormal wears or marks on anything? I hope not an internal failure.

If a clutch is not properly disengaging or is dragging in any way, it will make shifting very difficult when the engine is running as the lack of clutch disengagement is putting some load on the tranny gears. When you shut off the engine, the loading on the tranny gears goes away. If you think about it, since the engine is no longer spinning and trasnferring a load thru a partially disengaged clutch the trans is not loaded up, making it easier to shift.Bank of SHO:
Does anyone have an answer to this question: With the ignition off, it shifts just fine; does this mean anything or help the diagnosis?
