Three adjustments on two cars: Maybe six-eight total shims needed to be replaced. I only had to order two, as I was able to swap shims around and had a couple of spares. Both cars were '89s.

Very typical. When I did my last valve adjustment. Everything was in spec. So I wrote down all the sizes of the shims and flipped them over to give them a new wear surface.

4-5 (not by much). No idea if it had ever been done in the previous 298k miles

yup, common that they are all in spec. I think out of 4 engines I have done, the only ones I needed to change were when I changed to 3.0 intake cams, and even then typically only one or two needed to be adjusted.

Nope, that's common. In fact, only 1 of 3 SHOs I've owned and done top 60's on had some out of spec. That was my 89, and it only had 2, I believe. One was very minor .001 or .002 out; the other one was out a bit more. I ordered what I thought was the correct shim and it was still out of spec, dug around in my garage and found 1 even thicker than the one I bought, installed it, and I was STILL out, but only by .001 or .002. I let it ride like that without any further issues.

Not sure if the miles had anything to do with it, but the 89 had the most miles on it by far compared to the others I've owned. I have 259k on it now!

I have done several valve jobs and seen it range from not needing anything to needing almost every shim. It just depends.

I think I may have had 1 out of spec when I did my initial on my car after buying it. I did swap shims around to tighten up already in spec gaps. Shortly after, I went through several sets of cams and played the shim hoarding game.

With the motor I built about 9k miles and 5 years ago, I just recently did a valve adjustment as I wasn't sure which way the valves would wear most. I thought they'd wear into the seats as well settle with the new shims/lash caps/etc. IIRC, I put them in the middle of the allowable clearances when I built the motor and only two valves (both #4 intake) were just a touch too large for their gaps. Everything else was in spec, however, I think all the gaps opened up just a tad.

I think the worst one I had was an exhaust at .015 which still isn't bad. Doesn't flipping them over wear the numbers off the shims?

EXACTLY. I swapped out that 1 because it was out of tolerance by a few increments. The others were not enough of a difference to matter; not on a 259k motor anyway.
