I have a 94 MTX SHO with 167,000 miles. Every time I shift it into fifth gear the RPMs shoot up to 5000. Is this a sign that my transmission is about to go out?

It seems to be a sign of the clutch going out.

It should do this each time to shift too soon and you try to floor it.

I second that. the motor revs because the clutch is slipping. ive seen this same thing happen on my freinds motorcycle when the clutch was going, it started with 5th gear and slowly got worse in other gears until he finally did something about it.

Sounds right to me too! Oil on the grab surfaces can do it too, but either way it has to come out.
I have never had a clutch plate outlast the "throw out bearing / pressure plate finger" wear event. Tell me how you did it!
Be prepared to tighten or replace the rear main seal, get your exhaust bolts ordered and check around for new/used mounts for the motor and tranny...... and be prepared for the subframe bushing replacement excitement, if your unit has not been lowered in awhile.
