So I decided to replace the subframe bushings and the rear passenger bushing was completely loose. The subframe is completely rusted out. (see attached photo) Has anyone ever repaired this by welding a plate to the frame? Or where can I find another subframe?

best bet would probably be a junkyard. you might want to get a rust free one sourced from a southern state if you're planning to get/keep the car rust free.

You can weld a plate in there if you had to.
I have seen it done.
I would recommend a different sub frame, though.

You could consider lobbing-off that ear and have a good welder attach another good ear off a junk yard car. We don't know what sub frame you have, but that mounting ear looks like a standard part for all Taurus sub frames near the year that you have. Take a look at the rest of the sub frame as it may be OK (or not).
You can get another sub frame from this forum, but shipping gets expensive.
