Do you think it came loose because of play in the pump shaft? It seems to be fine when trying to wiggle it by hand and the pulley seems to be good(not moving side to side). How tight should it be?

Are the belts in decent shape and tight? Most belt squeals are from loose belts or worn belts that no longer grip.

The belt is brand new, do you have to tighten it enough to compress the washer? Should the washer be on the outer part of the pulley?

Yes, the outside of the ps pulley should have a flat washer and a lock washer. You need to tighten the nut enough that the lock washer is compressed flat.
