Help diagnosis hesitation/miss/surging

Seems like there have been a lot of posts with similar subjects lately. Here is my situation. I have noticed that when the car is started after sitting for a while that it sounds like it is missing when the idle levels off even though the car seem to start fine. When driving the car it seems to hesitate or miss and then finally surge when the car reaches around 3000 RPM's. Above 3000 this problem doesn't seem to exist. Once the car warms up none of this is noticeable except when hitting a hill at low RPM's, then the same behavior returns. I recently completed a full 60K service including new wires (Taylor ThunderVolt 8.2), plugs (Denso) and coil pack. The car was having severe power issues recently and was throwing a bunch of lean codes. This was eventually fixed by replacing the fuel filter. The only codes I am getting now is during KOER and that is a 13 (IAB?). I did a cylinder balance test and it is indicating a problem with cylinder 3. From what I found it seems like most people think it could be a plug or wire since it isn't throwing any other codes. I pulled the plug and it looked a bit whiter then I would have thought for a new set of plugs. Is it possible the plugs were ruined by the lean condition? Any other ideas are appreciated. Thanks!

im in the same boat man. no codes though, plugs look great, passed cylinder balabce test. idk whats goin on

I'd check out your MAF. I had sort of the same symptoms and once I replaced my MAF they went away. I also went ahead and cleaned my IAC valve. Wouldn't hurt to look into these.
