While driving with the headlights on, after different intervals of time, the headlights just go out. This includes the low beams and fog lights, and also the high beams. Some times the lights come right back on, sometimes they don't. Sometimes switching the headlamp switch off and then back on lights them back up, but sometimes it does not.

If I pull back on the turning signal shaft to the headlight "flash" and hold it back while the headlights have gone out, the headlights that switch controls come on.

Bad headlamp switch, bad ground, bad relay, what? Any ideas?

The headlamp switch is prone to over heating. Pull it out and look for a burnt connector. Good reason to have a relay system.

There was a recall on the 93, 94 Taurus for the switch. 98S01
