Car problems with codes

My cars check engine light came on and off today while driving. I go home and pull codes. I got 511(have LPM for 80mm MAF), 126(Cylinder identification circuit failure). The CID is new. I then ran the test with the engine running and it gave me a 111(system pass) and then I tried to do the cylinder balance test by pressing the gas pedal and the car threw code 538(Insufficient RPM change during KOER Dynamic Response test). Then the car seemed liked it was missing. Shut the car off and restarted and she ran fine. I tried the tests again and it did the exact same thing. It won't let me do the cylinder balance test. I have done a complete 60k with Tim(Toolman as the master). Also I get bad mileage with about 18 MPG where ever I go. It smells rich sometimes. I have not gotten any oxygen related sensor codes. This is weird problem and just wanted some advice from the experts.

I would remove the LPM and try a stock MAF and see how it runs, at least remove the LPM.

STOP!

go back to shotimes and read the procedure for the koer test again, so you know what to expect.

when running the koer test, the engine will always sound like its missing - that is how it does the cylinder balance test to know if each cylinder if firing properly.

read the procedure and follow it exactly and it will work for you.

Sorry. I have done this test on many ford vehicles and I know what your talking about when it sounds like it is missing because it is testing the cylinders. What I meant to say it won't perform this test at all. When I press the gas to start the test, it won't do the test and immediately throws the 538(Insufficient RPM change during KOER Dynamic Response test) every time.

The 538 is because it isn't detecting enough throttle change when you goose it. Goose it all the way to the floor and watch your timing window.
