Power Steering Belt Separating?

I was diagnosing some noise that I was hearing as I would drive on my 94 MTX, it would change and get louder with the rpm's of the engine, kinda like a humming or a whine.

Tracked it down to the power steering tensioner pulley. The bearings were making all kinds a noise as I spun it on my finger. I noticed as I put the belts back on, the power steering belt, one of the ribs was separating from the rest of the belt. Not sure why that is... The crank pulley hasn't moved and the alternator belt is fine, no issues. There is no wobble at all to that ps tensioner pulley as the engine runs.

Not a huge deal, I can replace the belt no problem, just wonder what might be the culprit?

Any ideas?
