I purchased some moog strut mounts a month or so ago. Three of them, both the rears and one of the fronts, came with a small bag filled with 3 nuts and 2 somewhat bell-shaped washers. The other front mount, however, came with a bag containing only 3 nuts and no washers. Should all four mounts have these bell-shaped washers? Am I missing a set of washers or do I have one extra set? I'll take some pictures up when I get home to show exactly what I'm talking about.

Yep, all the mounts should have come with new washers. However, the washers arn't really a wear part. You should be fine just reusing the washers that came off the old struts in the corner where you don't have new washers.

I didn't get any new hardware with the Ford strut mounts that I just bought and installed. I tried to order new ones from Ford but they had to order them from Memphis and it would take 2 weeks (I'm in Canada remember). Kind of ironic since I was headed there and didn't have time. So I ended up paying a shop to disassemble the old struts and reassembling the new ones.

He's posting from Minnesota, those old washers may be a bit thin due to rust.

When i bought new Moog mounts for the GenI I got different parts most of the four boxes. Some had hardware missing and some had hardware and new bearings.
I got the parts from Rock Auto decided it was best to go local with this type of stuff in the future. Not RA's fault as they don't package the parts, but the shipping back and forth killed the $ I saved on original price.
