    Since you've ruled out many logical things, that leaves only out of the box things to check. I like rubydist's thought of the timing belt being off a tooth or so, or the cam timing being otherwise off. You could have an exhaust system restriction. You could have a strange intake restriction...

    You can check exhaust system back pressure with a gauge that taps into the 02 sensor bung. Are your fuel injectors the correct ones? We're running Denso O2 sensors, FWIW.

    The fan runs when the either the coolant temperature is high or the A/C is on, whenever the car is moving at less than 50 mph. If it's working in those conditions, I would not worry over what happens when the ignition key is cycled.

    I suggest getting a Lithium Ion jump pack, aka jump starter, to carry in the car for peace of mind until you get this sorted out. The cam sensor is relatively easy to change out, so it's a good place to start.
