messed up serp belt

I just installed a new serpentine belt on my 93 ATX because the old one was partially shredded. About 5 minutes after starting the car the new belt began to shred from the inside (closer to the motor). Then the belt actually jumped pulley channels on the Power steering, crank shaft and water pump pulleys. The pulleys look to be lined up and cannot see any obvious reasons for the jump or the shredding. The tensioner tightened up nicely and the bearings were tight and smooth (rolled nicely no slop) on all the idler pulleys, any ideas?

This just happened to me the other day. The crankshaft pulley actually came apart. Look down and see if the crank pulley lines up with all the other pulleys.

The crank pulley is actually two peices, and inner and outer peice, with rubber in between. Sometimes the rubber fails. You'll need another crank pulley.
