ATX to MTX proper swap

I know that I should search for this information and I did, but everyone has a different way of doing it. I know a few of you guys have already done this swap and was wondering if you guys could post the things needed to do a proper swap. Meaning that once everything is together it bolts up like stock and runs with no issues. I also know that I can't expect that there will be no issues but after searching for a few hours I wished that there was a sticky on how to do a proper ATX to MTX swap listing all the parts needed and what needed to be fabricated so that it would be easy to find. Anyways any help is greatly appreciated.
